What is ApoB?
Written by Dr Robin Wang
ApoB is a protein built into the particles that carry cholesterol and fat out to the tissues. Each particle carries exactly one ApoB molecule, so the amount of ApoB in blood is a direct count of how many of those particles are circulating.
Why does ApoB matter?
It is the particles, not the cholesterol inside them, that lodge in an artery wall, so how many are circulating matters as much as how much they carry. When particles are small each one holds less cholesterol, so the same cholesterol is spread across more of them and the count runs higher than the cholesterol figure suggests.
How does ApoB relate to other markers?
LDL Cholesterol and Non-HDL Cholesterol are cholesterol measures on those same particles. Triglyceride-rich particles carry ApoB as well, so a shift in Triglycerides can move the particle count while the cholesterol measures stay where they were.
What affects ApoB?
Insulin resistance and excess body fat push the liver to release more of the small, ApoB-carrying particles. Saturated fat in the diet changes how many particles the liver assembles in the first place. On the clearance side, inherited differences in the liver's receptors set how quickly it pulls those particles back out of circulation.
Why Do Health tests ApoB three times a year
ApoB follows two things the liver does, making particles and clearing them, and both shift over weeks with diet, body weight and insulin resistance rather than with any single meal. Measuring about every four months shows whether a change in particle number has held from one part of the year to the next. This is Do Health's monitoring schedule, not a general recommendation for everyone.
