FT4 is the hormone the thyroid releases. The test measures the part of it not attached to a carrier protein, which is the part that can enter cells. TSH comes from the pituitary and tells the thyroid how much hormone to make.
Why does it matter?
The thyroid sets the pace much of the body runs at. Too little of its hormone and a person feels tired, cold and slowed down; too much and the opposite. TSH shifts before FT4 does, which is why it is the first test when thyroid disease is suspected.
